If I had to guess, I'd say it's probably a nicely-done photoshop (or similar software) image. Anybody else have any info?

I've tried. The best I've been able to come up with are some very poorly inpainted face swaps on some existing pictures. The AI models in their current form are barely able to handle proper human anatomy, let alone complex contorted positions. Perhaps if someone was able to feed one of the models a huge collection of imagery showing off various poses that are used in autofellatio, it could produce something.

No Ai involved to my knowledge. I got to see a little bit of how he layers his projects which was cool. Seemed very time consuming & gave a really cool result/final product!

I would really like to try to create photos or images of autofellatio or other types of porn with AI, what do you recommend? searching on the internet I saw that BestVRPornSites has a free artificial intelligence sex site list but I didn't really understand how they work and what I can do about it. What do you think, are they valid? Do you know any others?
